Kathie’s story: Therapy helps after Parkinson’s diagnosis

The diagnosis of Parkinson’s disease in April 2017 was quite a shock to Kathie Bernstein. 

“At the time, my only symptom was a left foot tremor — no big deal,” she said. “At first I didn’t believe it, but by the time I got home, it hit me and I was terrified.”

Kathie didn’t have a family history, although a cousin would be diagnosed later, but even so she learned no two cases are alike.

“I had seven years of mild symptoms, but it began progressing in January 2024,” Kathie said. “I began to notice my voice getting lower and I was having more trouble walking and getting up from low surfaces like the couch, so I started having occupational therapy rehab again late last year.

“I have a very supportive husband who drives me to rehab.”

At Northside Hospital, Kathie has taken part in specialized therapies called LSVT BIG and LSVT LOUD.

LSVT BIG trains people with Parkinson’s to use their body more normally, improving movements for tasks as small as buttoning a shirt or as large as getting up from a chair or maintaining balance while walking. LSVT LOUD is a speech treatment for people with Parkinson’s and other neurological conditions.

Jaimi Carhart, supervisor of outpatient rehabilitation services at Northside Forsyth, said the therapies may help anyone with Parkinson’s, but it usually makes the most impact when started early or when there are minimal to moderate functional deficits present.

“Patients may receive the LSVT BIG or LOUD programs separately or together based on needs,” Carhart said. “Essential components of both programs are follow-through, practice and carryover to home.

“Those who continue their exercises after completion of their therapy program are able to maintain residual benefits as Kathie has.”

Kathie said her therapists “are great and focus in on what problems you are having,” and that’s helped her have more motivation to do the LSVT exercises.

“My walking and balance have improved,” she said, “and I can stand up without using my hands now!”

Kathie has also been able to meet a goal of returning to the gym and walking 15 minutes on a treadmill. She reports improved voice projection and the renewed ability to go shopping and to the movies.

“My experience at Northside has been a great one,” she said. “Each time when I walk in, I am greeted by name at the front desk and everyone is so nice.”

Prospective patients must obtain a referral from their physician and may benefit from completing both the BIG and LOUD programs together, if indicated. Referrals should specify LSVT and Northside OP rehab.
